This form is to be completed by the Ministry Supervisor of the Veta Morphus student(s).

Dear Ministry Placement Supervisor,

You will have been asked to complete this online Ministry Supervisor Application Form by a Veta Morphus student who is currently enrolled in the 11236NAT – Certificate III in Christian Ministry and Theology *. This qualification is delivered through Evolation Learning Pty Ltd RTO #45219.

The course has the following components:

• Bible Engagement
• Christian Community
• Mentoring
• Ministry Placement
• Peer Groups
• Retreats.

The Ministry Placement requires the student to be engaged in the following:
Major Ministry Placement: 40 hours
Short Term Ministry Placement: 16 hours

Your role is to oversee their work in the Ministry Placement component of the course, ensure they are engaged in appropriate tasks and help them reflect theologically on the ministry.

Please ensure that you read the Ministry Supervisor role description in the Ministry Supervisor Manual and complete and submit this Application Form, and attach a copy of your state required Child Protection Screening Check (Working With Children’s Check, Blue Card or other state specific document). If you have not received a copy of the Ministry Supervisor Manual with this form, a copy can be found at this link
The student will also provide you with a link to the Organisation Approval Form. Please ensure the authorised person at your organisation completes the Organisation Approval Form to confirm your role and the organisation’s responsibility regarding the student’s Ministry Placement. This might be you, your Senior Pastor, a Board Member or some other authorised person in the organisation.

If you are a Mentor and not a Ministry Placement Supervisor to the student, please complete the Mentor Application form.

Before you begin, please ensure you have the following information:

• The first and last names of all the students you will be supervising. If you offer to supervise others at a later date, you will need to complete this form again for those students
• Are you the Ministry Placement Supervisor for the student’s
  • Major Ministry Placement (40 hours) OR
  • Short Term Ministry Placement (16 hours) OR
  • Both (If you will be their Ministry Placement Supervisor, you will need to complete a separate form for each Ministry Placement, Major and Short Term)
• The Ministry Supervisor Manual and have read through it
• Have read the Veta Youth Policies and Code of Conduct

You will also need to arrange a meeting with the student(s) to complete the Induction Process using this Induction form. Please ensure this is completed as soon as possible.

You are required to provide to us a copy of your “Working With Vulnerable People” card.

NT Ministry Supervisor requirements

You are required to provide to us a copy of your Working With Children (WWC) clearance which is also called an Ochre Card. Please note that an Ochre card is valid for two years from the date of issue.

Victorian Ministry Supervisor requirements

You are required to provide to us a copy of your “Working with Children Check (WWCC) card” or Teacher Registration Card. Just quoting the WWCC card number will not be acceptable.

Western Australian Ministry Supervisor requirements

You are required to provide to us a copy of your “Working with Children Check (WWC) Card”. Just quoting the WWC Card number will not be acceptable. Please note that a Teacher Registration document is no longer acceptable.

Queensland Ministry Supervisor requirements

You are required to provide to us a copy of your “Blue Card”. Just quoting the Blue Card number will not be acceptable. Please note that a Teacher Registration document is no longer acceptable. However you can apply for and provide an Exemption Card.

Tasmanian Ministry Supervisor requirements

You are required to provide to us a copy of your “Working with Vulnerable People" (RWVP) Card. Just quoting the RWVP Card number will not be acceptable. Please note that a Teacher Registration document is no longer acceptable.

South Australian Ministry Supervisor requirements

You are required to provide to us a copy of your Working with Children’s Check confirmation email. (All pages of the WWCC email must be included). Just quoting the WWCC number will not be acceptable. Please note that Police Checks or Teacher Registration documents are not acceptable.

New South Wales Ministry Supervisor Requirements

You are required to provide Veta Youth with your NSW WWCC Number as part of this application.
